Association between Serpin A12 Levels with Some Hormones and Biochemical Factors in Polycystic Ovary Syndrome Women

Abstract

Serpin A12 is one of serine protease inhibitor (serpin) family, it is a protein secreted from visceral and subcutaneous adipose tissues of adult humans and that its secretion may be related with factors of obesity (fat content), and insulin resistance.  Polycystic ovary syndrome(PCOS) a disease affects the function of a woman's ovaries, and often its associated with hyperandrogenism and chronic anovulation. PCOS is also linked with a distinctive metabolic disturbance (resistance to insulin act) and fat content(obesity).  

This study is designed to detect if there is a relationship between Serpin A12 and Luteinizing hormone (LH), Follicle stimulating hormone (FSH), Prolactin(PRL), Testosterone, Estradiol, fasting plasma glucose (FPG), HbA1c and lipid profile in patients with PCOS. This hospital-based comparative study was carried out in Al-Batol hospital in Baaquba city/ Diyala governorate. sixty(60) women with PCOS, and thirty(30) healthy women(control group) were enrolled in this study (subdivided into two subgroups "normal weight and obese" according to the BMI) in order to evaluate the parameters of study.

Hormones are estimated in serum of patient by using an automated quantitative COBAS 411. Serpin is estimated by using ELISA kit. HbA1c is measured by a fluorescence Immunoassay(FIA) for the quantitative determination in human whole blood. Glucose, total Cholesterol(TC), Triglyceride(TG) and high density lipoprotein(HDL) were measured by using DIRUI Auto-Chemistry Analyzer CS-1240. Low density lipoproteins (LDL) and very low-density lipoprotein(VLDL) levels were calculated according to friedwalds formula.  

The results revealed an elevation in FPG and HbA1c in PCOS patients. FPG significantly increased in PCOS patients of the two groups (normal weight and obese) comparing with those of control group, but HbA1c is non-significantly increased in both groups.  The cholesterol, triglyceride, LDL, and VLDL are increased significantly in both patient groups comparing with control groups, while the HDL levels non-significantly decreased in both patient groups. The results of hormones that included in the study revealed a significant elevation in the two patient subgroups in comparing with control subgroups. Serpin has significant positive correlation with BMI and Testosterone. Serpin  has positively non-significant correlated with FBS, Cholesterol, HDL, VLDL, LH, Prolactin, Estradiol, HbA1c, TG, LDL ,and. In contrast, serpin is negatively correlate with FSH.

Conclusion: Increasing serpin levels in blood of PCOS patients. The elevation attributed to the accumulation of fats  in body or around the waist of patients lead to increasing BMI. Sex hormones takes a part  in the onset of  PCOS where there is increasing in androgens to estrogens and this stimulates ovarian stromal hyperplasia. Serpin has positive significant correlation with BMI and Testosterone of women with PCOS. Serpin is positively connected with most parameters of PCOS.
